Enrollment is an ongoing process, and we’re here to support you every step of the way.

Step 1: Complete the Gio’s Garden Interest Form

This form helps us learn more about your child and your family’s needs.

Step 2: Complete the Registration Packet

The registration packet includes forms required by state licensing. We understand this paperwork can feel overwhelming, and help is always available.

If you would like assistance completing the forms, please email middleton@giosgarden.org. We are happy to:

  • Complete the paperwork together

  • Answer questions

  • Schedule a meet-and-greet before all paperwork is finished

Step 3: Submit Forms & Schedule a Meeting

Once completed, registration materials for BOTH locations can be:

  • Emailed to Middleton@GiosGarden.org,* OR

  • Dropped off at our Middleton location*:
    2028 Parmenter Street, Middleton, WI 53562

* NOTE: Even if you are applying for the Sun Prairie location, please drop off forms at the Middleton location or use the Middleton email address.

After forms are submitted, families will schedule and attend a meeting with our Family Care Coordinator.

During this Meet and Greet:

  • Your child will explore Gio’s Garden

  • We will discuss any specific care needs

  • We will identify goals staff can incorporate during your child’s time at Gio’s

  • A start date and schedule will be determined
